Changing Your Company
A customer made transfer Lifeline benefit to another company once every 60 days for their telephone service and
for Internet service, once every 12 months.
Customers can transfer their benefit sooner under the following conditions

The customer has moved to a new address

The company no longer offers any Lifeline service

The company has charge late fees which are greater than the customers
out-of-pocket cost monthly for their service
Transferring a Lifeline Benefit
If you wish to transfer your benefit to another company, you need to contact that company that offers lifeline
and then you will ask them to transfer the lifeline benefit to them. you will need to send them the following
information so the process can be completed

Your name

Your date of birth
-
The last four digits of your Social Security number

Your address
-
Your phone number
-
A written or verbal consent from you
You need to give your consent which acknowledges that once a transfer is complete then you lose the benefit with
the company you worked with previously.
You have to acknowledge that your new company has explained that the customer may not have more than one
Lifeline benefit in their household.
most cases, a customer does not experience any interruption in their service. In some cases, the customer may be
required to provide Proof of Identity.
